Step 2: You will complete an (Initial Screening) Inquiry Form over the phone.

Step 3: Wait to be contacted regarding eligibility and space availability.

Step 4: If your family qualifies, you will complete a face-to-face interview.

Step 5: If your family is accepted into the program, make an appointment for intake.

* This process will normally take 1-2 weeks.

How To Succeed At Family Promise*

  • meet with Family Support Team members on a regular basis
  • report progress on the goals from your service plan
  • continually work on your goals as defined by your service plan

* A family's stay in the program is contingent upon whether they follow the program guidelines and work actively on goal attainment. All school-aged children must attend school.
